- Submit a recent athletic physical conducted within the last two weeks.

- Disclose any physical or mental limitations, such as ADHD, depression, or other diagnoses, and provide supporting documentation.
Identification
- Provide a copy of a valid identification document (passport, state-issued driver's license, etc.).
Health Insurance Coverage
- Submit proof of current health insurance coverage.
Deposit
- A $500.00 deposit is required to secure your spot.
Contract of Financial Obligations
- Sign and submit the contract outlining any other financial obligations.
